The play described below is provided for your review and discussion. The rulings given are based on NFHS rules. NCAA rulings may be different.
As A-4 and A-5 are running up the court, they are yelling at each other over a botched play that just happened. A-4 gets pissed off enough to decide to take a swing at A-5. A-5 responds by throwing his fist at A-4. Eventually cooler heads stop the melee.
Handle the situation.
By definition, this is fighting. Both A-4 and A-5 are charged with flagrant personal fouls and are ejected to the bench. Team B will be awarded four free throws and the ball at the spot nearest to where the fight broke out. If such spot is in the frontcourt, the throw-in will be from nearest designated throw-in spot.
Since no player on Team B was fouled, Team B may designate who the free throw shooter will be.

Very Interesting Play. A similar situation happened to me a couple years ago. During a Timeout,
two teammates started fighting (boxing match). They were brothers and the Dad came on the court
and left with them. We ended the Game, then as the Team had only four Players and behind by
25-30 points. Thanks!
Laughable, a 25 point lead can be overcome within a quarter. Lest you forget that a team may continue play with merely one player…… and your game should not have ended by an official calling it. Learn the rules and stop responding with your comments henceforth!
