There is a loose ball that results in a tie-up between A-1 and B-1.  After the whistle has sounded, A-1 gets up and throws the ball at B-1.  B-1 retaliates by throwing the ball back at A-1.

A-1 has four fouls.  Three are personal fouls and one is a technical foul for unsporting conduct.  A-1 is whistled for a player control foul.  Immediately after the whistle has sounded, A-1 is charged with another technical foul for unsporting conduct.

Team B scores with 14 seconds remaining in the game and pulls to within 2 points of Team A.  Team A is in no hurry to pick up the ball and only begins the throw-in when there are 5 seconds left.  The ball is inbounded and A-1 is fouled with 1.5 seconds to go.  

How should this situation have been handled?

A-2 is attempting her last free throw with 2.6 seconds on the clock and her team trailing by one point.  After the release of the free throw but before the ball hits the ring, A-1 crosses the three-point arc.

A-5 scores.  B-5 recovers the ball and heads toward out of bounds to throw the ball in.  However, B-5 never steps out of bounds and passes the ball to B-1 who starts up the court.
