A-5 receives a technical foul for unsporting conduct.  Later in the game, A-5 receives a technical foul for hanging on the rim.  A-5 leaves the game.  The officials fail to notify the Team A head coach that A-5 has been disqualified.  Two minutes later A-5 is back in the game and is fouled during the act of shooting.  At this point, the table reminds the officials that A-5 had received two technical fouls.

Handle the situation.

A-1 is fouled while dribbling the ball.  The foul puts Team A in the bonus but the table fails to notify the officials of this.  Team A inbounds the ball and A-2 attempts and makes a three point goal.  Team B picks up the ball for the throw-in and begins the throw-in.  Then the table sounds the horn.  Play is stopped and the officials learn that A-1 should have been awarded a bonus free throw situation.

Handle the situation.
